Laurel Ridge Rehabilitation and Skilled Care Center

Ascentria Care Alliance, Inc. (“Applicant”) located at 14 East Worcester Street, Suite 300, Worcester, MA 01604 intends to file a Notice of Determination of Need (“Application”) with the Massachusetts Department of Public Health for a conservation project by Lutheran Home of Jamaica Plain, Inc. d/b/a Laurel Ridge Rehabilitation and Skilled Care Center (“Facility”) located at 174 Forest Hills Street, Jamaica Plain, MA 02130. The Application requests approval for renovation and construction to the Facility to come into compliance with applicable regulatory requirements in 105 CMR 150.000: Standards for Long-Term Care Facilities requiring long-term care facilities to de-densify three- and four-bedded resident rooms to private and two-bedded rooms (“Proposed Project”). The total value of the Proposed Project based on the maximum capital expenditure is $12,739,902. The Applicant does not anticipate any price or service impacts on the Applicant’s existing Patient Panel as a result of the Proposed Project. Any ten Taxpayers of Massachusetts may register in connection with the intended Application by no later than October 28, 2021 or 30 days from the Filing Date, whichever is later, by contacting the Department of Public Health, Determination of Need Program, 250 Washington Street, 6th Floor, Boston, MA 02108.


Quaboag Rehabilitation and Skilled Care Center

Ascentria Care Alliance, Inc. (“Applicant”) located at 14 East Worcester Street, Suite 300, Worcester, MA 01604 intends to file a Notice of Determination of Need (“Application”) with the Massachusetts Department of Public Health for a conservation project by Fair Havens, Inc. d/b/a Quaboag Rehabilitation and Skilled Care Center (“Facility”) located at 47 East Main Street, West Brookfield, MA 01585. The Application requests approval for renovation and construction to the Facility to come into compliance with applicable regulatory requirements in 105 CMR 150.000: Standards for Long-Term Care Facilities requiring long-term care facilities to de-densify three- and four-bedded resident rooms to private and two[1]bedded rooms (“Proposed Project”). The total value of the Proposed Project based on the maximum capital expenditure is $6,111,556. The Applicant does not anticipate any price or service impacts on the Applicant’s existing Patient Panel as a result of the Proposed Project. (“Applicant”) located at 14 East Worcester Street, Suite 300, Worcester, MA 01604 intends to file a Notice of Determination of Need (“Application”) with the Massachusetts Department of Public Health for a conservation project by Lutheran Housing Corporation-Brockton d/b/a Lutheran Rehabilitation and Skilled Care Center (“Facility”) located at 26 Harvard Street, Worcester, MA 01609. The Application requests approval for renovation and construction to the Facility to come into compliance with applicable regulatory requirements in 105 CMR 150.000: Standards for Long-Term Care Facilities requiring long-term care facilities to de-densify three- and four-bedded resident rooms to private and two-bedded rooms(“Proposed Project”). The total value of the Proposed Project based on the maximum capital expenditure is $7,450,550. The Applicant does not anticipate any price or service impacts on the Applicant’s existing Patient Panel as a result of the Proposed Project.
