Individual Service Options in New Hampshire

Ascentria Care Alliance offers individual service options [ISO] tailored to meet the individual needs of children and families. These intensive level services can be delivered in biological, foster, adoptive or relative care homes. These services can also be delivered to older adolescents who are practicing to live independently. Referrals for this service are accepted from the Division for Children Youth and Families and the Division of Juvenile Justice Services.

Child Health Support Services

Ascentria assists families with parenting skills and helps to establish a safe and positive home environment. These services may include parent coaching, behavior management, nutrition training, parenting-child communication skill building and other community resources.

 

ISO In-Home

ISO (Individual Service Options) in-home services usually last up to six months, though this can be extended with agreement from the treatment team. This service can be used to help strengthen families and prevent removal from the home or can be used to assist in the reunification process. Services delivered to the home include individual/family counseling and support, 24/7 on call emergency support, respite, crisis management, treatment plan development and implementation, transportation, advocacy with schools, health providers and other community resources and assistance with any other identified needs.

ISO Foster Care

Ascentria licenses foster homes that are available to serve children with intensive level needs. Foster parents help to develop and implement the treatment plan, may supervise visits with birth families, provide transportation to various appointments and meet the daily needs of the children placed in their home. Ascentria relies on foster parents to display appropriate modeling and teach skills to help the foster child reach his or her potential. Foster parents are reimbursed at a daily rate and are required to complete 24 hours of training per year. Any child placed in an Ascentria ISO foster home will receive services that can include individual/family counseling and support, 24/7 on call emergency support, respite, crisis management, treatment plan development and implementation, transportation, advocacy with schools, health providers and other community resources and assistance with any other identified needs.

ISO Independent Living

Independent living ISOs are ideal for youth who are aging out of the foster care system. It is for youth who need to learn the necessary skills to live independently, but it has been determined that foster care may not be the best option for that particular youth. Ascentria is committed to thinking “outside the box,” to create individual service options that will meet a youth’s particular needs. This can include providing a roommate to help a youth gain those skills or providing a basement apartment in a licensed foster home. Any child in an independent living ISO will have a case manager dedicated to teaching and then overseeing those independent living skills as the child transitions in to adulthood.

Home Based Therapeutic Services

Home Based Therapeutic Services [HBTS] is a short term (up to 90 days) service that provides home based therapy to prevent removal of children from the home or can be used to assist with the reunification process. Clinicians work closely with families to build on strengths in order to enhance parenting skills, encourage self sufficiency and assure safety. Clinicians are on call 24/7 provide crisis management. Referrals are made to community providers who can provide support and assistance after the HBT services end. This is a less intensive service that does not include transportation, respite, supervised visits or the presence of clinicians at school and other provider meetings. If a family is in need of these other services then a referral to ISO in home services is most appropriate.
